Collection and consent statement - healthcare practitioners
- The services and/or products provided by Mindset Health Pty Ltd ABN 11 617 368 957 (Mindset, we, us, or our) include digital hypnotherapy programs for irritable bowel syndrome (Nerva), menopause (Evia), smoking cessation (Finito), mental health (Claria), and chronic low-back pain (Relio).
- In handling your personal information, Mindset complies with the Privacy Act 1988 (Cth) and all other relevant Australian privacy and health records laws.
- All collection, use, disclosure and storage of your personal information by Mindset will be for the primary purposes set out in our Privacy Policy which can be accessed https://www.mindsethealth.com/legal/nerva-privacy-policy including:
- providing our services and products to you and patients that you refer to us;
- communicating with you and responding to any inquiries you may have regarding our services and products;
- providing educational marketing material about our products;
